Summary of Pertinent Case Features: The patient’s main features were self-destructive behaviours and impulsivity when she first came to seek for the assistance.
Presenting Problem: She came alone so seek for the assistance and she reported frequent feeling of anxiety. She came alone because she felt that she still had control over the problem and did not require to be brought by anyone else for the help. She also pointed out that she felt detached from herself and often feel into depression. At some points she would forget relevant things that were facing her. She pointed out that she would like to be helped from the current problems so that to coexist with others in a more friendly manner. The problem presented by the patient can be controlled through a diagnostic process that involves step by step handling of the problem.
Medical History: The patient has not undergone through a previous medical treatment process.
History of Mental Illness: The patient has not been treated for any mental problems previously.
History of substance use/ addiction: The patient had undergone through an alcohol dependence treatment program and had successfully recovered from the problem.
Stressors in past year: In the past year, the patient lost her job because of a misunderstanding that occurred between her and her employer. She has been employed in a new job but she fears that her condition might result to her losing the new job. She is also facing the problem of unstable financial situation that occurred when she lost her first job.
Safety assessment: She denied of a possibility of committing suicide or harming herself because of the situation.
Appearance/ behavior: She looked anxious of the things around her which includes our interview. She also looked disorganised
Thought Processes: I also noted that the patient appeared paranoid of issues and people around her.
Mood/ Affect: At some point, the patient appeared happy then all over a sudden her mood changed because of small issues within our conversation.
Intellectual Functioning: According to our conversation she appeared intelligent but clouded by her current condition.
Orientation: I am an experienced counsellor who is devoted to his work. The patient’s case was oriented to her condition on the place.
Final Diagnoses: She was suffering from Dissociative identify disorder which can be diagnosed through a talk therapy which often takes place for a duration of two months.
