This article bases its discussion on the value of self-management of chronic disease. It further broadens the understanding of whether there is the need for more innovation and utilization to improve health outcomes and the reduction of emergency room visits. This is a vital source of disease management is not only a health care provider initiative as people can also conduct their disease management. As a result, this article is very effective in understanding the roles of individuals in their care and the management of their health programs. For my research paper, this a very useful article as it consolidates all types of care and opening up ways through which individuals can actively participate in disease management thereby improving health. I will use this article to elaborate the benefits of disease management and primarily individualized management for both the individual and the society as a whole as well as in prevention of disease spread.

Disease management takes on several forms within a health care facility. This book is valuable in highlighting these facts. For one it talks about the various programs as well as the outcomes of these programs. The programs highlighted in the book are bundled payments, Medication Management, and Accountable Care Organizations. In addition to this information, the book further introduces other concepts of disease management such as clinical foundations, opportunity analysis and the use of propensity matching. These are all components of good disease management by bodies and facilities. With this, the book is a resourceful item in covering the various parts and elements of disease management. The book will, therefore, play a critical part of my project as it will expound and give additional knowledge of the factors that make up disease management. I will specifically use the information on the book to create a section for the programs that are critical in disease management.

Self-management approach is a critical form of disease management style. In as much as this is the case, there are occurrences when the impacts of these programs are not felt, and instead of having a positive outcome, they may have elements of negativity. This article relates the account of the inefficiency of self-management programs in managing of certain chronic diseases and arthritis. It further exposes the faults that may occur in implementing the program. The article gives a fresh angle to the issue as not always does the program work as it is only natural that there could be something that affects its eligibility. Through assessing the difficulties, it is possible to formulate future solutions to the issue of self-management of diseases. I, therefore, will use the information to facilitate in the recommendation of what can be done to ensure that disease self-management is a success.
