IHP 435 Milestone Three Guidelines and Rubric

Overview: Healthcare administrators must be able to understand and effectively use performance-improvement methods to further institutional goals and remain compliant with regulatory standards. The administrator role in healthcare organizations may vary, but the administrator’s ability to recognize the role of data in establishing meaningful thresholds for patient safety and quality and ability to analyze information to develop performance-improvement activities are essential. Your final project for this course will require you to place yourself in the role of a healthcare administrator with the responsibility of performing a quality assessment on your organization.

Prompt: Your topic for your final project will be a study of readmission trends for SNHU Hospital. For comparison and informational purposes, you have been given a data set representing readmission trends for the entire state of New Hampshire and SNHU Hospital. The third table in your data set shows where patients were readmitted after they were discharged from SNHU Hospital.

In this milestone, you will draft the third part of your final project (Section III, Parts A–C), which includes basic data analysis. You will analyze the data provided, consider common cause and special cause, discuss frequency distributions and ratios, and speculate about risk factors.

Below are some ideas to consider as you work. Note that these are guiding questions to assist your analysis. Your paper should include the critical elements below.

  • What assumptions can you begin to make from the data?

  • What do the Medicare numbers tell you?

  • Are there any numbers that jump out that would indicate special cause versus common cause?

  • What internal (institutional) factors and policies have an impact on readmissions?  What external factors have an impact on readmissions?

Include the following critical elements:

  1. Are the variations in the data common cause or special cause? Explain what information you can gather from the data.

  2. Describe the data using frequency measures (as practiced in the Module Four Activity). Be sure to include frequency distributions, ratios, and proportions when appropriate.

  3. Based on the data you analyzed and the supplied information, what potential risk factors might need to be considered? In other words, what might be problematic for the institution moving forward, based on the data? What challenges might they face? Be sure to consider both internal and external factors that have an impact on readmissions.

Guidelines for Submission: Your paper should be submitted as a 2- to 3-page Microsoft Word document with double spacing, 12-point Times New Roman font, and one-inch margins. Citations should be in APA format.
