Intro:

Also called a wellness program, health promotion programs offer senior citizens benefits that aim to maintain as well as promote their health and welling. Some may include those as free check-ups and immunizations, smoking cessation training, nutritionist visits, and guidance on healthy living and lifestyle or cooking as well as free cholesterol testing and management. Various health promotion programs play an essential role in helping the elderly adopt and maintain healthy self-managing behaviors and routines. They also help in reducing health service utilization as well as increasing their well-being. It is so vital given that the elderly are most affected by chronic health conditions or experience behavioral issues as depression and anxiety.

Aging and Disability Resource Centres (ADRCs) are national initiatives and works to meet the needs and wants of the elderly. They can go to these centers when in doubt or when they have questions on how to get placement or to know what Medicare pays for. They can also learn a lot from these centers on how to get services in their homes and how to get other products at a lower fee without having to depend on their children necessarily. In other words, it navigates these people in the right direction and also caters to the needs of those disabled (18 years and above).

Patient-Centered Medical Homes (PCMHs) have teams of professionals working with the elderly as well as covering them throughout their treatment. While they put these patients at the center of care, they offer care coordination no matter where the patient may be. This model of care is all about wellness and prevention, as well as anticipating patient needs (Goetzel, Loeppke, Ballard, & Serxner, 2014). The elderly form a relationship with the nurses and doctors, social workers, pharmacists, and together they are involved in the elderly patients' care and a plan to get them better. As a result, there is more personal time spent with the doctor to care for patients' needs and requests personally. The ultimate goal, however, is a healthier patient. Notwithstanding, the team of professional cares for the patient in their hospital, a different hospital, or at home.

Senior centers offer socialization opportunities to senior citizens. They provide activities and gatherings for elderly fun and entertainment. Such centers provide the seniors with a chance also to stay active and stay healthy as they also take trips to visit the doctors. Meals are provided daily, and this cuts costs and activities needed in making food at home (Baicker, Cutler, & Song, 2010). The senior care programs offer independent living, companionship assistance, and nutrition to the elderly, thus improving thousands of lives in the community.
