You work for a large city hospital. In this hospital, there are:

  • two primary care outpatient clinics (pediatrics and adult)

  • four specialty clinics (orthopedics, neurology, psychiatry, and ophthalmology)

  • 300 inpatient beds

  • an in-house laboratory

  • a radiology department

  • a pharmacy

The hospital medical record consists of paper charts. The hospital wants to implement an EHR, and it is your job to develop a proposal to present to the hospital board.

Your proposal should cover the following:

  1. Type of EHR (Turnkey or Best of Breed) and why, 1 page.

  2. Functions the EHR will have and why, 2-4 pages.

  3. Benefits that will be expected from implementation of this EHR proposal, 2-3 pages.

TOTAL: 5-8 pages.

Final Review

There is certainly a lot to learn about the Electronic Health Record! In this class we discussed the many functions an EHR can perform, and you were able to try many of them out in the Lab Activities. This week you will be bringing everything you have learned together into a Final Project, developing a proposal for an EHR. As you do this assignment, think through all you have learned in the past weeks.

Types of EHRs

One thing that has not been discussed yet is the different choices when choosing an EHR. Here are two options to consider.

Turnkey - This means that the entire EHR is provided by one vendor or brand.

Benefits to Turnkey are:

  • All functions of the EHR are guaranteed to work well together.

  • Support is easier because it all goes through one vendor.

  • Costs may be less because everything comes through one vendor and one contract.

Drawbacks to Turnkey are:

  • There may be some functionality an organization wants that is not offered by the vendor.

  • If something negative happens with the vendor, it will affect the entire EHR of the organization.

Best of Breed - What is meant by this is that the organization chooses what they feel is the best vendor for each function, so there are many different vendors and brands all piecemealed together.

Benefits to Best of Breed are:

  • An organization can choose what they feel is the best fit for them for each function.

Drawbacks to Best of Breed are:

  • Interoperability may be a challenge. One vendor's product may not interact well with another vendor's product. Costly interfaces will be needed, and those interfaces will need to be maintained.

  • This can be a more expensive choice. Contracts with each vendor must be maintained.

  • Support can be a challenge because support staff will have to learn about many different types of products rather than just one.

  • If something technical goes wrong, it can be challenging to figure out what the cause of the problem is. Vendor A will say that it is Vendor B's fault and vice versa.
