Patient Name: Juan Duran, Mexican American, currently visiting diabetes clinic at the VA, He is exceedingly polite and respectfully healthcare personnel’s. He does not like to interrupt too many questions. He has been USA more than 50 years and navy veterans. Duran was wondering around the hallway and found by Mr. Carson clinical director. Mr. Duran tells him that he’s been instructed start insulin for his diabetes but does not know how. He does not receive call or call for any instructions. Mr. Carson finds diabetes nurse educator Jenny O’Conner and asked her for Mr. Duran’s scheduled for appointment.
Nurse jenny interviewed Mr. Duran.
Mr. Duran Married, lives in Chula Vista, CA in a comfortable apartment with his wife. Speaks fluent English, but wife’s English is limited. Speaks Spanish at home. Limited vision, wife must administer insulin. Had one daughter, who was found murdered in the apartment parking lot less than two weeks ago, leaving Mr. and Mrs. Duran to care for her child. Mr. Duran seems befuddled by the insulin and syringes and is stoic when he talks about the loss of his daughter. Mr. Duran is a diabetic male who has been instructed by his physician that he needs to start insulin for his diabetes, however, he does not know how, and does not recall receiving any appointment or instructions. After being referred to the nurse educator, the nurse interviews Mr. Duran and learns that he is confused by the insulin syringes and has limited vision; therefore, his wife administers his insulin. The nurse educator also learns that Mr. Duran’s daughter was recently found murdered in her apartment building and he is now caring for her child. The two theories that are most related to this case study, and which will be helpful for the nurse educator to use in order to assess Mr. Duran and develop a care plan would be the Neuman systems model and Orem’s self-care deficit nursing theory. Neuman’s theory focuses on the human needs of protection or relief from stress (McEwen, & Wills, 2014).


Orem’s model deals with self-care (PP142-146), and Mr. Duran will need to care of his diabetes with his wife’s help. When using Orem, the nurse interacts with the medical team to plan and administer care and assists the patient and family in learning to care for themselves. The model also includes the family and culture within the environment (P.144). For Neuman’s model (PP 149-154), ideas about the role of stress are vitally important with a patient like Mr. Duran. In addition, the five interacting variables: physiologic, psychological, sociocultural, development, and spiritual-all will be important in his care.
Based on Dorothea Orem’s theory, what additional information would Nurse Jenny want to collect /assess: for Orem’s theory, jenny would want to assess the Duran family’s current knowledge of diabetes, diet and self-care. In addition, she would be concerned about aspects of his environment such as family support a culture.

Comparison of the Case Study Lessons to the Related Theories The theories related to the case include Neuman’s framework and Orem’sself-care concept. Neuman’s concept focuses on the need for a dynamic balance among individuals. Medical professionals are responsible for providing equity by establishing challenges, similar goals, and prevention measures.
