Rationale for Assessment:

To conduct a self-rapport, measure the intensity of physical and cognitive anxiety in the past week.

Background Information:

Jasmine is a 30-year-old black female who is seeking therapy for couples counseling. She is currently employed at Holiday Inn Hotel here in Beaumont Texas. Jasmine reports that she has good family support and has no problems getting along with family and friends. Jasmine presented problems with non-communication in her current relationship with her partner. Jasmine also reports that she is unsure about marriage because the trust has been broken in the relationship. Jasmine reports that she is motivated and willingly to fix issues in the relationship, and wants to be able to effectively cope with her anxieties and constant worrying about her future with her partner.

Observations:

Jasmine appeared to be oriented to person, place and time and was well groomed. She spoke with intelligence and understanding and appeared to comprehend what was being said to her.

Discussion of Results:

Jasmine has a raw score of 41 and T score of 109 percentile rank of 99.8 and diagnostic range as sever: The client expresses a high level of symptomatology associated with anxiety. Her anxiety may significantly interfere with information-processing functions and result in poorly planned responses to environmental pressures. The inability to meet life's demands adequately may then exacerbate the client’s real-world problems and lead to greater anxiety and even greater difficulty in responding to environmental pressures. The counselor will make anxiety-reduction strategies the primary focus of treatment. However, I will need to also examine the client for depression using the Beck Depression Inventory II or other appropriate assessment procedure, because of the high rate of co-morbidity between anxiety and depression.

Clinical Implications and Recommendations:

Jasmine appeared to be eligible and appropriate for therapy and was recommended 1x weekly over the next 6 week for 1.5 hours per session.
