Quality improvement does not occur overnight and rarely occurs without systematic and systemic guidance. Healthcare settings are complex, involving numerous stakeholders, regulations, technologies, and patient needs. Thus, change efforts in healthcare are problematic if not approached in a purposeful, organized manner.

In this assignment, you will select a quality improvement (QI) model to direct implementation of the plan you created in your Module 3 assignment.

Step 1. Select
Select a quality improvement (QI) model appropriate to guide the implementation of the plan you proposed in your Module 3 assignment to improve care of older adults. Outline implementation steps in alignment with the QI model.

Step 2. Review
Review the table “Quality Assurance vs. Quality Improvement” (Table 2) in the Practice Facilitation Handbook linked on the Learning Objects page. Consider the implications of both quality assurance and quality improvement in the issue, topic, or condition you selected.

Step 3. Create
Using the resource linked on the Learning Objects page or a design or tool of your choice, illustrate implementation of your plan as aligned with a quality improvement model. Include annotations indicating quality assurance and/or quality improvement related to the table you studied in Step 2.

Step 4. Explain
In a 1-2-page paper or sentence outline, explain how you could use the infographic with an interdisciplinary team to guide implementation of your plan.

No quality improvement initiative occurs in a silo; every action needs support and produces both positive and negative reactions. For any change initiative, leaders must deploy appropriate leadership strategies, engage, educate, and train all stakeholders, and measure results to ensure fidelity of implementation of strategies and quality of outcomes.

In this assignment, you will build upon the work you have done in previous assignments to outline a professional staff development session for healthcare workers engaged with older clients.

Step 1. Identify
Identify an audience of professional (and possibly support staff) healthcare workers needed to implement your plan for improvement in the quality of care for older adults.

Step 2. Articulate
Articulate the specific purpose for a training session aimed at improving processes and protocols according to evidence-based practices. This purpose should directly relate to the issue, topic, or condition you addressed in your Module 3 assignment.

Step 3. Outline
Outline an evaluation plan for your quality improvement plan and how you will share actions and responsibilities with the healthcare team.

Step 4. Reflect/Analyze
Conduct a SWOT (Strengths, Weaknesses, Opportunities, Threats) analysis regarding implementation of your quality improvement plan, including your interaction with individuals and groups in the training session. Consider the resources required for the training and implementing the plan and types of leadership strategies needed to leverage the opportunities and overcome the threats.

Step 5. Outline
Outline a full-day training session for appropriate healthcare leaders and workers in preparing to implement your quality improvement plan. Using any format you choose, include objectives, a timeline, activities, materials, and how you will evaluate the effectiveness of the session. Plan for a variety of activities to keep the participants engaged and focused upon objectives and improvement outcomes.

Step 6. Conclude
Write a 4-minute speech or presentation to conclude the training session. Your motivational concluding speech should address these topics:
