Scenario 1 - Mrs Chan

Mrs Chan lives in a residential aged care facility. Mrs Chan has several medical conditions listed below.

You are on duty and doing patient care rounds of a night shift and find Mrs Chan on the floor in her private room dressed in a night gown. The last thing Mrs Chan remembers is getting up to the toilet and to fill the vase with water several hours ago, she said that she fell on her way to the toilet. She states she has pain in her right hip area and the right leg is rotated internally. Mrs Chan has a large piece of broken glass from the vase in her left thigh.

Mrs Chan’s nightgown is wet with urine and there is a pool of urine on the floor.

Past Medical History:

  • Urinary incontinence,

  • Osteoporosis,

  • Old compression fractures of her spine due to osteoporosis and

  • Chronic pain

Regular Medications:

  • Fosamax

  • Vitamin D

  • Calcium

  • Fentanyl patch

  • PRN Endone

  1. Describe the process of completing a primary survey on Mrs Chan.

The primary survey is completed following the DRABC acronym:

D – danger – check for any dangers that makes the surrounding hazardous. As there are broken glasses, the area needs to be cleaned so the environment is safe.

R – responsive – check if the patient is responsive or unresponsive by using the COWS method (can you hear me, open your eyes, what is your name, squeeze my hand). Mrs. Chan is responsive, and she is able to answer questions.

A – airway – you need to check that the airway is open and clear. If the airway is blocked, you can open airway by tilting the head back and use two fingers from the other hand to lift the chin.

B – breathing – check for any abnormal breathing by watching their chest movements, listening for sounds, and feeling the movement of air as they inhale and exhale.

C – circulation – check for any signs of severe bleeding. In Mrs Chan’s case, she may be bleeding severely as she has an embedded piece of glass. This can be treated by using dressing or clothing to reduce risks of hypovolemic shock, and it is important to act careful not to move the embedded glass as it can cause additional trauma.

Explain 2 specific assessments that would assist you to evaluate Mrs Chan’s penetrating wound on her left leg.

  1. Review the attached vital signs (ADDS) chart and evaluate the following

  1. List each of the vital signs recorded at 0630 in the table below:

List the vital sign

  1. Complete the ADDS score for each vital sign recorded at 0630 in the table below, then total the ADDS score.

  1. Describe the 2 abnormal signs using correct medical terminology.

Respiratory rate (normal 12-20 beats/min)

– her rate is 25, it is a condition known as tachypnoea

Temperature (normal 36.1 – 37.5 degree Celsius)

– her temperature is 38.7 degree Celsius, a condition known as hyperthermia

Scenario - Further Information

Mrs Chan is seen by the doctor, who documents her assessment as follows:

  • 82 year old lady with confusion and hyperthermia

  • Air entry R=L no adventitious sounds

  • Large laceration to her left thigh, glass remains imbedded in the wound

  • Right leg has obvious bone deformity- requires further investigation to evaluate possible fractures.

A chest, abdominal, pelvis and hip X-ray are to be completed. Intravenous normal saline 1L is prescribed over 4 hours.

  1. a) Noting the ‘Actions Required’ section of the ADDS chart, you are to explain who should accompany Mrs Chan when she leaves the ward to go to the X-ray department.
