Your Practicum experience includes working in a clinical setting that will help you gain the knowledge and skills needed as an advanced practice nurse. In your practicum experience, you will develop a practicum plan that sets forth objectives to frame and guide your practicum experience.

As part of your Practicum Experience Plan, you will not only plan for your learning in your practicum experience but also work through various patient visits with focused notes as well as one (1) journal entry.

Part 2: Individualized Practicum Learning Objectives


Refer to the instructions in Week 2 to create individualized practicum learning objectives that meet the requirements for this course. These objectives should be aligned specifically to your Practicum experience. Your objectives should address your self-assessment of the skills found in the “PMHNP Clinical Skills Self-Assessment Form” you completed in Week 1.

As you develop your individualized practicum learning objective, be sure to write them using the SMART format. Use the resources found in Week 2 to guide your development. Once you review your resources, continue and complete the following. Note: Please make sure each of your objectives are connected to your self-assessment. Also, consider that you will need to demonstrate how you are advancing your knowledge in the clinical specialty.

** YOU MUST HAVE 3 NEW OBJECTIVES EACH QUARTER. You may include previous practicum objectives; however, you still must have 3 new objectives for your current course.

Objective 1: <: Improve Formulating Diagnostic Assessment according to DSM 5-TR with supervision from course preceptor.

Specific: Focus on improving my ability to perform diagnostic assessments with limited supervision as per the DSM 5 TR. Measurable: Perform at least 5 comprehensive diagnostic assessments every week and utilize EBP, consult course preceptor and peers. Achievable: Integrating feedback and using validated assessment tools will enable to gain independence in performing diagnostic assessment confidence with limited supervision or independence. Relevant: Mastering diagnostic assessment is crucial for my professional development as a mental health practitioner: Time-bound: Complete the planned activities every week for the next four weeks.

Planned Activities:
Planned to perform at least 5 comprehensive diagnostic assessments on my patient end of the week under the supervision of my course preceptor.

  • Seek feedback review from course preceptor and to identify areas of strength and areas that need improvement because the focus is to be able to perform diagnosis independently without supervision.

  • Use evidence-based assessment tools specific to mental health to enhance diagnostic accuracy.

Mode of Assessment: The mode of assessment that will be employed include

  • Direct observation and feedback from the preceptor

  • Having a weekly reflective journaling on assessment experience

  • Submission of the identified case studies demonstrating the application of the assessment tool.

PRAC Course Outcome(s) Addressed:

  • Develop a professional Plan in Advanced nursing practice for the practicum experience.

  • Familiarize myself with the criteria found in the DSM-5 for psychiatric diagnosis and differential diagnoses.

Objective 2: < Want to improve psychopharmacology knowledge and application.

Specific: Focused on improving my understanding of psychopharmacology, especially pharmacokinetics and pharmacodynamics. Measurable: I will track my progress by participating in 12 weekly medication management sessions and researching and summarizing evidence-based material. Achievable: These activities are achievable within my current schedule by utilizing available resources and getting guidance from my preceptor. Relevant: This objective is directly related to my role and responsibilities and so it is important to have proper psychopharmacological knowledge and confidence to prescribe independently. Time-bound: The mastery and confidence needed to prescribe independently will be achieved after 10 weeks.

  • Planned Activities: Participate in weekly medication management sessions with a focus on understanding pharmacokinetics and pharmacodynamics in different population.

  • Research on evidence-based material on psychopharmacological treatment and associated case studies on complex patient scenarios.

  • Consult preceptor on selection and management of psychotropic medication, side effects, and education strategies.

Mode of Assessment: The mode of assessment includes

  • Peer feedback on case study presentation and drug recommended.

  • Coming up with reflective journal and progress notes

Synthesize theoretical knowledge with clinical practice in the management of

psychopharmacological treatments

Objective 3: < Improve Therapeutic Communication Skills

Specific: By the end of the practicum, I aim to improve my therapeutic communication skills. Measurable: The mastery will be demonstrated by the positive patient feedback I get. Achievable: I will achieve this objective by participating in relevant training sessions and applying all learned techniques in clinical practice. Relevant: Effective therapeutic communication is crucial for patient care and outcomes. Time-Bound: Achieve this by the end of the practicum.

Planned Activities:

  • Engage in role-playing exercises to practice therapeutic communication techniques.

  • Conduct patient interviews focusing on building rapport and trust.

  • Seek feedback from patients and the clinical supervisor on communication effectiveness.

Reflect on patient interactions and identify areas for improvement in bi-weekly journal entries.

Mode of Assessment:

  • Supervisor evaluations are based on direct observation of patient interactions.

  • Patient feedback surveys.

  • Review of reflective journal entries by the clinical supervisor.

  • Keeps a reflective journal documenting communication experience and lessons learned, to be reviewed weekly.

Part 3: Projected Timeline/Schedule

Estimate how many hours you expect to work on your Practicum each week. *Note: All of your hours and activities must be supervised by your Preceptor and completed onsite. Your Preceptor will approve all hours, but your activities will be approved by both your Preceptor and Instructor. Any changes to this plan must be approved.

This timeline is intended as a planning tool; your actual schedule may differ from the projections you are making now.

I intend to complete the 144 or 160 Practicum hours (as applicable) according to the following timeline/schedule. I also understand that I must see at least 80 patients during my practicum experience. I understand that I may not complete my practicum hours sooner than 8 weeks. I understand I may not be in the practicum setting longer than 8 hours per day unless pre-approved by my faculty.
