Medication Administration Prep Sheet [MAPS]


Directions: Look up the assigned medication in the Drug Handbook and Pharmacology Text. Mark the page for easy reference. Read all the information concerning the medication. Complete the MAPS. List the trade and generic names. List the classification and the action of the medication (how this medication works). List the reason your patient is receiving this medication. List the most common nursing implications or care required in the administration of this medication (before, during or after).

Colace

(Docusate Sodium)


Stool softener- promotes incorporation of water into stool making it softer, to prevent constipation. Patient immobile due to fractured hip.

Assess for abd distention, bowel movement, diarrhea.


Lasix (Furosemide)


Loop diuretic- inhibits reabsorption of sodium and chloride from loop of Henle and distal renal tubule, increases renal excretion of water. Diuresis and mobilization of excess water (edema, pleural effusion) Decreases BP. Patient has history of CHF.

Assess serum potassium (causes hypokalemia), BP, edema, lung sounds, electrolytes.

Administer in AM.

Heparin


Anticoagulant-prevents the conversion of prothrombin to thrombin and fibrinogen to fibrin to prevent thrombus (clot) formation DVT prophylaxis, atrial fibrillation

Assess for bleeding (gums, epistaxis, bruising, tarry stools, hematuria, dec. H & H, dec. BP). Do not aspirate.
