Colace (Docusate Sodium) | Stool softener- promotes incorporation of water into stool making it softer, to prevent constipation. Patient immobile due to fractured hip. | Assess for abd distention, bowel movement, diarrhea. |
Lasix (Furosemide) | Loop diuretic- inhibits reabsorption of sodium and chloride from loop of Henle and distal renal tubule, increases renal excretion of water. Diuresis and mobilization of excess water (edema, pleural effusion) Decreases BP. Patient has history of CHF. | Assess serum potassium (causes hypokalemia), BP, edema, lung sounds, electrolytes. Administer in AM. |
Heparin | Anticoagulant-prevents the conversion of prothrombin to thrombin and fibrinogen to fibrin to prevent thrombus (clot) formation DVT prophylaxis, atrial fibrillation | Assess for bleeding (gums, epistaxis, bruising, tarry stools, hematuria, dec. H & H, dec. BP). Do not aspirate. |
