Who are we surveying and what will be the purpose of the survey?
The survey would be administered to the employees and managers who frequently deal with organizational information during decision-making at the workplace. This group is suitable as it can give first-hand information in respect to the interpretation, trust and application of data in actual workplace settings. The survey would assist in further understanding of the practical functionality of data driven decision making, perceived benefits, challenges and organizational influences. The results might be used to determine the gaps between organizational expectations and actual data use.

Is there a particular choice that has been informed by data and what was the result?
What do you think are the reasons that help you or hinder your capacity to use data to make critical decisions?
The approach to take while writing the questions differently.
The initial open ended survey question is on the daily usage of data in decision making. The second question makes the participants think about actual situations when data contributed to the results. The third question investigates organizational or personal aspects, which affect dependence on data. These questions are intended to gather qualitative responses in detail and meaning.
Had these questions been prepared as a focus group rather than a survey, they would be given in terms of inviting interaction and dialogue among the participants. The questions would be more general and followed up prompts would help to compare experiences and discuss divergent opinions. The discussion would be moderated by a protocol that would help reduce bias, control the voices of dominant participants and make everyone contribute. This would focus on group thinking and not individual thought.
