Selecting the Aggregate


 Capstone Project: Overview



The Capstone project is a multipart project that spans ten weeks. The goal of the Capstone project is to select an aggregate (community or group of people), assess its health needs by conducting a risk assessment, develop a care plan for the aggregate and describe how you would implement the plan, and evaluate its effectiveness. Each week, you will complete one phase of the project, building upon the work completed in the previous weeks. Your completed Capstone project is due in Week 10.

You will be required to select an aggregate (population) to work with for this class; however, you will not be able to select a client population from your place of employment. You may choose your community, county, city, or another group or organization; e.g. a senior citizen's center, a school or a particular grade level in a school, a faith based group, a support group, or a group from your local public health department.
