1. Louise is a 35 year old female.  Her husband noticed that about one week prior to being evaluated, she started sleeping only three hours a night.  At about the same time, she started shopping online and spending money the family did not have buying jewelry and clothing that she did not need.  He was very concerned since she has always been very frugal and not very interested in fashion.  However, when he tried to talk to her about his concerns, she told him not to worry since she felt great and nothing bad could happen to her.  Louise had continued to work without problems at her job at the local library and seemed to be taking adequate care of their two children who attend elementary school.  During the evaluation, Louise did not present with evidence of psychotic features or depression. Her speech was normal in rhythm, rate, and volume.   Her mood did appear elevated and she said that she had never felt better in her life.

2. Carl is a fifty year old male who has had a history of distrusting others since he was a teenager.  Although he does not experience delusions or hallucinations, he continually thinks that others are trying to harm and deceive him.  At this point, he has few friends, because he has always doubted their loyalty and gets angry easily when he thinks they have betrayed him.  Despite his friends’ attempts to reassure him, he still distrusts them and never forgets any perceived disappointment or slight.  Over time, most of his old friends have tired of constantly being falsely accused and have limited their contact with Carl as a result.  Carl thinks that his current isolation is the result of their betrayal and attacks on his character.

3. Judy is a 16 year old female who is constantly in trouble for stealing. Judy steals things at school, from her family, and in stores. The objects that she steals are not things that she needs and she does not sell
them to makes money.  She reports that she sees something and cannot resist the urge to steal it even though she knows she will get in trouble.  She says that she experiences an emotional “rush” when she steals and really steals for that feeling.  She has no other behavior problems and no known medical conditions.

4. Tina is a thirty year old female who has a lengthy history of psychiatric hospitalization.  Over the last ten years, she has asked to be admitted to the hospital for suicidal ideation, cutting on herself, and low lethality suicide attempts (such as taking six Tylenol and then immediately calling 911).   Tina has been a series of unstable relationships and tends to request hospitalization when each relationship ends and she feels abandoned. In all of the relationships, she tends to vacillate between idealizing and then hating her partners.  She reported that she was sexually abused as an adolescent and has been sexually promiscuous since that time. She tends to
drink heavily on the weekends and at that time often sleeps with anyone that shows any interest in her.  She also states that she just does not know who she is or what she should do with her life.  She has never been employed for any extended period, but has a history of many different entry-level jobs.

5. Angela is a fifty year old female.  She married right after completing high school and has never lived alone.  She does not like being alone and spends all of her day with either her mother or her husband.  Despite her age, Angela has no confidence, she lets her mother and husband make all of the decisions in her life.  She is very afraid of conflict and will not disagree with anyone.  She does not believe that she can take care of herself and worries all of the time that she might someday be alone.  Her husband and mother decided that she would not be able to care for children, so she never had children.  She tends to do whatever they tell her to do and begs them to never abandon her.  There is no evidence of psychosis and her cognitive abilities are within the normal range.
