While taking an advanced class at the State Fire Academy I was assigned to the truck during a multi-story training fire. My job assignment was outside ventilation. Upon arrival, we received reports of a victim from the third story window on side "C". With the help of the roof man, we took a 35 foot ladder to the rear. I stepped onto the rungs to heel the ladder for the throw. The fly section was not properly secured and slid onto my foot. With my feet trapped, we could not complete the operation. We needed assistance removing the ladder from my feet.

Lessons Learned

Ensure the ladder is properly bedded before deployment.
Never step on the rungs to heel the ladder.
