Directions on how to submit your grant. While you are “writing” your grant, please be sure to SAVE OFTEN. You don’t want to risk losing an entire document you haven’t had the chance to save.

You will be uploading a series of documents and will upload ALL of them prior to submitting.

1A-Background and Needs Assessment

  • Overview of problem and data at a national level with supporting research, peer reviewed articles

  • Overview of population for programming, its data with supporting research, peer reviewed articles.

  • Community analysis- Use of PATCH or MAPP (what would be done to collect further data on your population AND supports the above.

  • Attach the Logic Model we started in class. It must be complete.

1B-Statement of Purpose

  • Summarize The purpose and/or goals of the grant(copy and paste) clearly taken from grant document

1C Work Plan/Implementation Plan

  • Identifies and explains at least one model that supports programming

  • Brief narrative of programming

  • Uses three column chart (objectives, strategies, assessment/evaluation)

  • Objectives are SMART, three columns align with each other

  • Everything aligns with the grant

  • The work plan (what is done, by whom/by when, follow up) with the follow up column blank

  • Your excel spreadsheet is an appendix…that shows the work plan in a spreadsheet

1D-Evaluation Plan

  • Brief evaluation narrative that summarizes the above charts

  • Includes samples of evaluation tools (2-4)-can be appendices

2A-Capacity and Organizational Support

  • Narrative describes needed staffing, equipment and other resources (succinctly and thoroughly) a paragraph.

  • Describes an existing strong relationship with the population

  • Describes an existing relationship with other organizations that would be involved with the planned program.

  • Advisory Board- a list of partners (organizations and people) that/who support the grant/program

In a grant-appendices do not count in your page count…but they do have page limits as well.
