By the end of this activity, you will be able to:

  • Describe the health care external environment

  • Compare and contrast commonly-utilized tools of environmental analysis

  • Explain how environmental analysis supports strategic momentum through validation of the assumptions held by management concerning their operational environment.

This case study illustrates a strategic change movement at a struggling teaching hospital in Massachusetts.  Tufts-NEMC engaged in a series of strategies to try to adapt to a rapidly changing health care environment. You will extract the salient strategies employed and analyze the effectiveness.

Read Case Study 6 – Ellen Zane – Leading Change at Tufts-NEMC.  Then, analyze this case by answering the following questions. Use the case study analysis rubric as a further guide to what is expected in your analysis.

This is your second graded case study. This case provides an illustration of the significant pressures faced by the management of a rural not-for-profit health care facility. In addition, the case provides the opportunity to review stakeholder analysis and stakeholder theory. The case attempts to provide a framework for you to debate the major strategic concerns currently faced by CEO Moen.

This is the third graded case study – Swayne, Case Study 9 – Governance Challenges at Good Hands Healthcare.  This case is designed to depict a series of governance issues for a health care company within the eldercare industry and to provide you a framework by which to link the results of internal analysis and the development of service delivery implementation strategies.

Activity Outcomes:

  • Analyze the results of internal and external analysis

  • Evaluate change analysis to determine ways in which competitive advantages are gained and disadvantages are improved

Answer the following questions regarding the Swayne Case Study #9:

  • Briefly describe Jackson’s assessment of Good Hands’ issues and proposed strategies.

  • Do you believe that Jackson’s assessment of Good Hands’ problems is a result of poor industry/external conditions?  Why or why not?  

  • Is Jackson the right person to lead a turnaround at Good Hands?  Why or why not?

  • If you were a board member, what alternative would you suggest at the next board meeting regarding Jackson?

Introduction

In this course, you are expected to write a 5-7 page research paper describing components of strategic change. You will base this paper on the Comprehensive Primary Care Initiative, a key component of the Patient Protection and Affordable Care Act.  This paper will be due at the end of Module 7. Details of this paper are found below.

This research paper is worth 35% of your overall course grade, so please start preparing no later than Module 2.

Paper Expectations

The Patient Protection and Affordable Care Act created the Comprehensive Primary Care (CPC) initiative, a multi-payer initiative fostering collaboration between public and private health care payers to strengthen primary care. Medicare will work with commercial and State health insurance plans and offer bonus payments to primary care doctors who better coordinate care for their patients. Primary care practices that choose to participate in this initiative will be given resources to better coordinate primary care for their Medicare patients. There are 500 primary care practices participating in the CPC initiative.  This represents 2,144 providers serving an estimated 313,000 Medicare beneficiaries.

Please choose a primary care practice in a geographic area of your choice.  Please write a double-spaced 5-7 page research paper responding comprehensively to the points listed below. This paper draws on cumulative knowledge attained to date in this course. You must cite your readings and at least two articles from the Excelsior College Library. All references should be formatted in APA 6 style.

  • Explain the history of patient centered medical homes and describe how that history has evolved to what is in place today.

  • Explain the strategic reasons for a primary care practice to pursue the development of a patient centered medical home. Think about what might be revealed in internal and external analyses.

  • Describe the implementation strategies that you would utilize.  

  • Describe at least three potential assets and barriers to implementation. Discuss what you would do overcome barriers you have identified.
