Visitation Questionnaire

Instructions: Prepare for your interview by choosing 5 areas of interest (out of the 10 listed below) in addition to the required sections (Introductory Information and Career Advice). Complete the questionnaire during your organization visit and save an electronic version on your computer. The completed questionnaire will be copied and pasted as an Appendix (supplementary information) when you submit your memo.

Note: Appendices usually appear at the end of a document as supplemental information, but are not essential for document completeness. Thus, the questionnaire is a tool for organizing your interview questions and is required as an Appendix, but the instructor will primarily grade your written memo.
