Policy Statement

[Aviation Company] intends to provide a safe environment for all employees, customers, and visitors. In pursuit of this goal, [Aviation Company] will maintain and abide by the following aviation safety policy. [Aviation Company] acknowledges that an active safety program requires the participation of every employee. It is therefore the expectation of every employee to strive for safe operations; to report accidents, incidents, and hazards; and to follow the policies listed in this document.

The [Aviation Company] Safety Team will be organized under a Director of Safety, who will report directly to the President of [Aviation Company]. The Director of Safety will coordinate with the Director of Flight Ops, the Director of Maintenance, and the Chief Pilot. Additionally, the Director of Safety will appoint Safety Liaisons inside the various operational departments within [Aviation Company]. They will do so in consultation with each department manager and the desired candidates themselves, to ensure meaningful selections. The Safety Liaisons will primarily continue their work within their department, but will have the additional responsibility of coordinating safety issues with the Director of Safety.

Authority and Responsibility

The General Manager, line managers, and employees at every level are responsible for reporting accidents and incidents along with prevention and implementation of the safety program. The following section documents the specific authorities and responsibilities given to the Director of Safety.

The Director of Safety has the authority to:

 Develop the necessary forms and instructions for implementing the aviation safety program

 Require the reporting of any aviation safety-related event

 Conduct aviation safety audits of any [Aviation Company] operation or facility

 Represent [Aviation Company] regarding aviation safety matters in dealing with government agencies and professional organizations

 Manage the budget supporting the safety program

The Director of Safety is responsible for (or to):

 The maintenance, review, and revision of this program

 Heading and organizing the safety committees

 Maintaining a reporting system for accidents, incidents, and hazards

 Distributing aviation safety information

 Conduct regular aviation safety committees

 Guide recording of aviation safety committee minutes

 Facilitating regular aviation safety audits

 Coordinating aviation safety training for new personnel

 Maintaining an aviation safety awards program

 Developing and maintaining a pre-accident plan

 Conducting accident and incident investigations

 Developing an aviation safety data analysis program

 Submitting an annual budget to support the safety program

Reporting of Accidents, Incidents, and Hazards

Accidents / Incidents: All accidental damage to [Aviation Company] aircraft, injury to [Aviation Company] personnel and customers from aircraft operation; or damage to [Aviation Company] property must be reported to the Director of Safety through appropriate means. All accidents and incidents will be documented and filed in the Safety Reporting System.

Occurrence: Anything that is not considered an accident or incident may be reported to department supervisors and/or referred to the Director of Safety as necessary. [Aviation Company] will work towards a resolution(s) as necessary to prevent future occurrences and further a safety learning culture.

Hazards: Anyone who witnesses, observes, or hears of hazards are encouraged to report by name or anonymously to the Director of Safety through the safety reporting system. Upon receiving a hazard report the Director of Safety shall investigate the reported manner and follow up as appropriate within 7-10 business days.

Distribution of Aviation Safety Information

It is the responsibility of the Director of Safety to distribute pertinent aviation safety material. The Director of Safety will distribute the information to the General Manager along with every head of each department. The heads of each department will then sub-distribute the information to all personnel of the department.

Aviation Safety Council

There will be a Safety Council established to coordinate safety among the various departments; find potential safety issues; implement agreed upon safety practices; and disseminate any pertinent safety information. Two committees shall be created: one for Flight Operations and one for Maintenance and Ground Operations. The Director of Safety will chair each committee, which will meet quarterly.

The Director of Safety will determine the agenda for each meeting, and the committee will submit recommendations to [Aviation Company]’s management. All committee actions will be by majority vote with the chair holding the tiebreaking vote.

Meeting attendance will be noted and minutes kept. The minutes will be retained by the Director of Safety and made available to any employee upon request. Employees may attend committee meetings with the approval of the committee members.

Aviation Safety Audits

Safety audits will be facilitated by the Director of Safety annually to track safety progress in both flight operations and aviation maintenance sectors. Safety audit records will be maintained by the Director of Safety and retained on file for a minimum of five years to view progress.

After the safety audit has been completed, an audit report will be generated and delivered to management and the safety committee. Items that the Director of Safety determines require immediate action will result in a meeting between the Director of Safety, the General Manager, and the head of the department.

Aviation Safety Training

The Director of Safety will develop an annual re-current training program for all new and existing personnel. This training program will consist of initial safety training course, and re-occurring safety training. The Safety Liaisons will track both initial and re-occurring training for each employee.

Aviation Safety Awards

[Aviation Company] Safety Awards will be given to employees who contribute significantly in reaching a high standard of safety within the company’s aviation safety program. The recipient of the award will be nominated by the manager of each department to the Director of Safety. The Director of Safety will decide if the nomination is appropriate and will report to the General Manager for final approval. The awards will be given publicly by the General Manager of the company.

Aviation Accident Investigation

The Director of Safety will develop and maintain an Emergency Response Plan as a separate document from this aviation safety program. This plan will be exercised or reviewed at least annually. The Director of Safety will maintain a roster of personnel in both flight operations and aircraft maintenance who are qualified to participate in an investigation.

An internal company investigation of any accidents or incidents will be conducted. This investigation shall establish the facts of the accident or incident and provide recommendations on eliminating or mitigating related future occurrences. These investigations shall be conducted by the Director of Safety, the Safety Liaisons for each affected department, and any additional staff that is approved by the General Manager per incident or accident.

The Director of Safety is authorized access to company records relevant to an accident investigation, and may interview any personnel related to the accident. The Director of Safety shall determine the retention period of documents, with approval of the General Manager.

The Director of Safety shall compile data from each investigation with their own findings, as well as those of the Safety Liaisons. The report will contain a summary of events leading up to, during, and after the accident or incident, the investigation team’s analysis and conclusions, and recommendations for future accident avoidance. This report will then be presented to the General Manager.

After the presentation of the report to the General Manager and upper management, and with approval from the affected parties, a mandatory Safety Council meeting will be scheduled with employees of the appropriate department to brief them on the accident and the means to prevent it.

Aviation Safety Analysis

In order to identify mishaps and trends within the Safety Program, the Director of Safety will maintain an analysis system using data compiled from safety reports, audit and investigation results. The Director of Safety will create and document a quarterly report detailing the results of the safety analysis program, which will be made available to the safety committee, managers, and executives.

Review and Revision

In order to remain relevant and to resolve and improve issues within the safety program, this policy will be revised at the discretion of the Director of Safety. Additionally, the [Aviation Company] Safety Program and this document shall be maintained by the Director of Safety. It will be reviewed annually by the safety committee, and all suggestions for revision are welcomed and should be forwarded to the Director of Safety. All revised versions will be signed by the General Manager of [Aviation Company].
