Circumcision is often done while newborns are still in the hospital. Different practitioners are trained to perform circumcision in newborns, including pediatricians and obstetricians. Most obstetrician and only one pediatrician do circumcision in the hospital I work. Right after delivery, a nurse from labor and delivery gets verbal consent from the mother if the mother wants her baby boy to get circumcised. If the mother says yes, the doctor will put an order in the computer system. Then the mother will be transferred to our floor (postpartum).  When we receive the mother as a new admission, we give her a paper circumcision consent to sign. Usually after 24 hrs. we contact the doctor who is going to do the circumcision.  Before the doctor perform the procedure, he will go to the mother and explain the risks. Once that is done, the nurse will take the baby to the nursery for the procedure. The doctor and the nurse must read the medical record number of the baby to make sure that is the right baby. Then the procedure will begin and the nurse assist the doctor with the procedure. When the procedure is completed, the nurse put the infant's name (usually mothers last name) and room number on a paper chart then someone from the billing department will collect those papers and turn them to a bill. The reason I choose this process is because in my opinion collecting papers and send a bill is very weak way of handling finance because paper could be lost or a nurse may forget to even put the name of the infant. 

I would like to see improvement such as: 

1. mother should give verbal consent after the birth of the baby. (system should show "yes For Circ.)

2. Mother should sign consent. (system should show consent signed)

3. Pediatrician should explain the procedure. 

4. Procedure completed.

5. system should show ( Circ completed ) then , the billing department can count the circumcision for that day. 

SMART

1. Nurse must receive verbal consent from mother 2 minutes after delivery of a baby boy. 

2. Mother must sign consent in 24 hrs.

3. Pediatrician must explain the procedure as soon as consent signed. 

4. Procedure must be done with in 10 minutes. 

5. procedure must be documented, system must show completed sign.
