  In the healthcare industry, pressure sores are a constant problem and challenge. One scale they use to determine if a patient is at risk for pressure sores is by using the Braden scale. Barbara Braden developed it in 1987 and it is widely used in more than 30 countries. Although the Braden Scale has been found to have better predictive validity than nursing judgment, the best care is prescribed when The Braden Scale is used in conjunction with nursing judgment. Some patients will have high scores and still have risk factors that must be addressed, whereas others with low scores may be reasonably expected to recover so rapidly that those factors need not be addressed (Adv Nurse 2006). 

     Good hand washing is another evidence base practice that proves to prevent the spread of infections. Jean Watson believes that when nurses make a conscious intention to be fully present to our patients, to intentionally bring whatever healing we can and to trust in the Life's Healing Power to meet the patient where we are insufficient, that holistic healing does occur. She advocates that nurses use intentionality to enhance nursing care (Watson 2002). So make sure you wash your hands before and after each time you come in contact with your patients.
