In an APA style essay paper, clearly describe and discuss why it is important to document. Appropriately utilize two sources besides your textbook to demonstrate your understanding.Be sure to cite all sources.

Address each of the following and its importance regarding documentation:

• Being a professional
• Basic legal aspects including state licensure requirements and HIPAA
• Personal aspects including liability, record-keeping, recall of sessions, demonstrating progress to the client and/or 3rd party
• 3rd party billing of insurance and/or personal injury
• Communication between Health Care Professionals, including how good documentation and communication helps reduce possible conflict
• Documentation in an evidence based practice or research case studies
