The concrete footing is H = 2 ft thick and B = 12 x 12 feet with an allowed stress of 3000 psi (note units). It sits on a heavy clay soil with an allowed stress of 6 k/square foot. You can omit the weight of the pipe and plate as trivial, but the weight of the concrete footing at 150 pounds per cubic foot must be accounted for.
