Include your greatest accomplishment; ( need you to make one up i did not include this )  a significant challenge and how it was addressed; and influences on your decision to become an OT. ( need more elaboration on why i want to be an occupational therapist) 

• Your personal qualities that will contribute to the OTA to OT program and the occupational therapy profession. ( MORE DETAILS TO ANSWER THIS ) 
•  write about an occupational therapy frame of reference in the paper too in relation to answer the questions.. IF you dont know what an occuaptioanl therapy frame of reference is look it up
