Hi I am trying to find a 3 good nursing diagnoses for a patient experiencing an acute exacerbation of heart failure. the patient has a history of congestive heart failure (CHF) and hypertension, constipation and other "heart problems". When the patient presented to the ED he reported fatigue, shortness of breath, especially at night and when walking. He also had swelling of his ankles and feet. He stated he did not take his "water pill" for the last five days because his wife's ankles were swollen so he gave the pills to her. He also admitted to being out of one of his heart pills but cannot remember which one.
