The interpreters receive training in interpreting and understanding medical terminology.

Family member or close friend to patient: use of family members or close friends provides both benefits and challenges. The benefit is that they are readily available and may better understand the patient’s culture which may be beneficial to the healthcare team. Their limitation is that it may compromise a patient’s right to confidentiality and they can also lack in training as interpreters thus leave out vital information when communicating with the healthcare team.

Nutritional screening represents the first line process of identifying patients who are undernourished or at risk of undernourishment (DeLaune & Ladner, 2010). It should be conducted within less than 24 hours of a patient’s admission. It is usually undertaken by nurses. It involves measuring a patient’s height and weight to determine their body mass index- BMI if it is less than 18 or above 40. It also gets conducted to determine presence of pressure ulcers and if the patient has problems with food intake. A care plan and regular monitoring can then be devised based by the RD dependent on if the patient is under high risk. Nutrition screening normally takes 10-15 minutes.

The Braden score is arrived at by examining six subscales. sensory perception, activity, moisture, nutrition, mobility, and friction or shear. Each subscale is rated on a scale of 1-4 with the exception of shear/ friction which gets rated at 1-3. Nurses conduct interviews and physical assessment to obtain data from each subscale. The total score for the six categories may range from 6-23 where a lower score may indicate a higher risk (DeLaune & Ladner, 2010). The following Braden score ranges determine the patient’s risk of pressure ulcers:

Nutrition is vital in the treatment of pressure ulcers. Protein energy under nutrition gets identified as a massive risk for pressure ulcers. Adequate
