2. Look through all grammar issues: Standard used is not correct (Standard issue.) I would also like for you to get (place) the employees that (who) serviced...

3. The focus and sim here is to find a way forward; to use the mistakes as a template for how to find solutions. Do you not have any suggestions in this context? A new software system? Some sort of checks and balances plan that could be implemented?
