The challenge for the community nurse will be to help the client understand the need to change their behaviors with eating and with exercise. In this scenario, the individual must start exercising daily and start eating meals at home instead of using take away food or restaurant food for most of his meals. He must also understand nutrition, how to read labels at the grocery store, and how to buy nutritious food for himself. In this situation, it seems that the Trans Theoretical Model (TTM) is the best to use because this individual must learn self-care.
In the first phase of education, the nurse would encourage this client to understand more about hypertension. Most important would be to explain what hypertension does and the consequences of not treating it. The first step would be a general overview of hypertension and its symptoms. Once these were discussed, the nurse would provide the answers to any questions that the client may have about their own hypertension.
