Follow the instructions in the manual linked below to create a template document for reporting a suspected or confirmed incident.

There is no template for this assignment, you must design a template that has professional formatting features and the appropriate content as described in the manual.

Start with creating a Policy Document to aid in decision making and rules for notification.

Second use your Policy Document to aid in the creation of a Notification Template for incident reporting.

Finally, use the documents to assist with a "Jump Bag". 

For class, the "Jump Bag" will just be a list of incident response materials.

In practice, the "Jump Bag" will be a physical and digital kit used for incident response. Keep in mind this is like a first aid kit and will need to be inventoried regularly to ensure the tools available are there and appropriate.
