There is a relationship between obesity, insulin resistance and development of cardiovascular complications. Adipose tissue is now being recognized as an endocrine organ, capable of secreting a number of adipose tissue-specific proteins, especially cytokines, known as adipokines, which have been implicated as a cause of endothelial injury and inflammation resulting in development of atherosclerosis and insulin resistance in obese individuals (Lau et al, 2005). Adipokines are also responsible for producing "the metabolic syndrome" which acts as a further predisposing factor for development of cardiovascular heart disease (Lau et al, 2005).

abnormalities like hyperglycemia, hyperinsulinemia, hypertension and dyslipidemia (increased levels of triglycerides, LDL and VLDL and decreased levels of HDL). The Adult Treatment Panel III (ATP III) criterion for diagnosing metabolic syndrome, (as cited Grundy, 2005) is shown in table 1. The metabolic syndrome is present when three or more of the five criteria are present in an individual.

normal fasting blood glucose levels and levels of glycosylated hemoglobin (HbA1C) 1% above normal through weight reduction and exercise program. The goal of the weight reduction program should be to achieve BMI in the range of 21 to 25 kg/m2. Desirable waist circumference for men is102 cm and women is 88 cm. The person should be encouraged to do at least 30 minutes of exercise, most days of the week.
