Phillip is dementia patient. Phillip has been in hospital for 1 week, during this time he has had his medications reviewed, being moved to voluntary treatment, has had minimal episodes of aggression and appears more coherent, he is now responding to conversation as he speaks English just not when he is in an agitated state, he is taking his medication, attending to personal cares most of the time, at times needs encouragement to do them and is occasional incontinent.
