2.  A synopsis of what happen

3.  What vulnerability was exploited

4.  Who or what organization was responsible

5.  What was the impact (physical, market, financial, etc)

6.  Has there been a recovery

7.  What efforts were used in the recovery

8.  Any other interesting topic(s) or piece(s) of information that you may find interesting
