One of the perils the article mentions is with the use of hand-off reporting, pointing out that if not done correctly can be associated with avoidable adverse events and near misses, due to inadequate communication. I know I have personally experienced this, after getting report from the off-going nurse, I enter the patients room to do my assessment and feel like I just report about a completely different patient, their IV is in a completely different location than what was mentioned in report, along with saying their skin was âintactâ while they actually have a wound or incision, +3 edema which was also never mentioned, saying their MRI showed ânothingâ when indeed it did not, I could go on and on.

It is necessary for all members of the health care team to be efficient in giving a hand-off report to another member of the team that is taking over the responsibility of that patient. Good communication skills and being thorough in your reporting is key to avoid mistakes that could lead to adverse outcomes for the patient. Many hospitals, mine included, are implementing a standardized, streamlined shift report system at the bedside which has patient safety as its primary driver. Vital elements of the bedside report include the oncoming and outgoing nurse visualizing the patient together, reviewing medication records, and physician and nursing orders, establishing patient goals for the shift, and inviting patients and their families to participate.

The foundation of any successful and effective team-based health care is having the members of the team focused on shared goals with supporting team-based institutional leadership that is fully involved and supportive. To achieve shared goals that are meaningful and robust, the patient and family must be integrally involved as members of the team developing, refining, and updating the goals. Clear roles, mutual trust, and effective communication among all the members of the team are also essential to get the work done and the goals met (Mitchell, Wynia, McNellis, Okun, Wee, Rohrbach, Von Kohorn, 2012).
