The preceptor asked me to verify the medication with one of the facility doctors before transcribing to MAR. I was informed that after I will finish transcribing medications to MAR, the preceptor will crosscheck it before faxing it to the pharmacist, before the arrival of the patient, and before the arrival of the med from the back up pharmacist.

I worked in the nursing home. The team that worked at the Jefferson provided a lot of support. Second day of the internship was more challenging. During morning report, I was informed that care plane meeting was to be held at 1300pm with one of the quest daughter who flew from California. My preceptor was sitting beside me for support and to fill any gap. I was very nervous at first but the RP was patient as I explained the side effects of the medication. The quest has right hip surgical due to recent fall so she was taking Coumadin 3MG alternating with 3.5mg with PT/INR being done every two weeks. Skin check was done to see if they were any bruising or tarry stools. During the A.M. report, I was informed that the patient fell during the night and that she was sent via 911 for evaluation from where she came back with a CT scan done with no new order given. Neuron check was indicated as Q 1 for 72 hours.
