Write a Vacation Permission Letter to the College Dean, English help

Hi, I need to Write a Vacation Permission Letter to the College Dean.

Content : 

I am Planning to go to my home country from March 3rd to March 30th. 

I have 2 Weeks Break after the end of this semester (end on March 5th) + I need Two More Weeks Permission.

I have Registered for the following Classes in next semester (Spring 2016)..  CIS 614 & CIS 692. 

But I am not able to view the professor's Name on it. So i am reaching you. and This is my 

last semester. I am doing well in studies upto now my GPA is 3.95/4. 

Reason : I want to see my family..

Note : Please take great care of grammar & everything. Letter should be in formal way. Dean should be impressed after seeing it.
