Social workers understand theories of human behavior and the social environment and critically evaluate and apply this knowledge to facilitate engagement with individuals, families, groups, organizations, and communities. In this capstone section of the paper, you will apply one human behavior theory and one practice model to a specific case from your field placement (or another approved case if different from Part 1).

Your paper should include:

Overview of the CaseProvide a concise overview of the case (maintaining confidentiality). You may use a different case than Part 1 if appropriate.

Description of the Theory and Practice ModelDescribe the human behavior theory and the practice model you selected. Provide scholarly citations indicating where you obtained information about each.

Application to the CaseExplain how you applied the human behavior theory to understand the client’s situation.Then, explain how you applied the practice model to guide your intervention or engagement.Demonstrate integration — show how the theory informed your understanding and how the model shaped your response.
