Hi, i need a paper for my med-surg class in nursing. The paper should:

The assignment requires  to select a disease process and write a 3-page paper using evidence based practice guidelines.  In the paper you are to discuss the promotion of health, control of the disease state, and or reduction of hospital re-admission in relation to the disease process you selected. (Think of this as what the evidence recommends your patient should know or do to keep them well in relation to the disease process you selected).

You are to write the paper utilizing APA format. The paper should be no longer than 3 pages in length (I will stop reading the paper after 3 pages). The paper must provide cited information from 2 articles which identify teaching topics or “pearls” of how to plan, discharge, and or provide teaching on a topic to prevent disease exacerbation, lessen complications, and or reduce re-admission rates for that illness.

The articles must be referenced within the text of paper (APA format), you are to identify at least 5 teaching points for patients with your selected concept. You must then discuss at least 3 potential barriers that may prevent the patient from following the recommendations.
