• Proactive scheduling: Have a dedicated care coordinator or staff member proactively call and schedule follow-up appointments for patients identified through screening.
• Use technology for timely follow-up
• Telehealth: Offer virtual follow-up appointments via video or phone call to increase convenience and reduce barriers for patients.
• Patient portal reminders: Use the patient portal to send automated reminders for upcoming appointments and anxiety-management resources.
