SUBJECTIVE: H.D. is an 82 year-old Hispanic male who presents to the office reporting problems with his memory. "I have been more forgetful lately". He is accompanied by his daughter who reports she is worried about him. She reports H.D. has been forgetful with routine tasks like not turning off the burners on the stovetop after he is done cooking or missing an appointment because he lost track of time. She has noticed his thoughts and movements appear slower or delayed. H.D. states his wife of 50 years passed away 6 months ago and the forgetfulness began shortly afterward. He thought the forgetfulness was due to the stress and sadness of her death but it hasn't gotten better. H.D. lives alone in single story condo and his daughter visits once a week. H.D. reports a history of hypertension and benign prostatic hyperplasia (BPH). He takes prescription hydrochlorothiazide 12.5 mg by mouth daily for high blood pressure and Tylenol PM (acetaminophen 500mg and diphenhydramine HCI 25 mg) one tablet by mouth as needed to help with sleep.
