SECTION I
Toms River is a township located in Ocean County of New Jersey. The township is believed to have begun as a result of the settlement of Thomas Luker in 1685, a man who ran a ferry across the Tom River. His name bred the “Tom” in Toms River. In the 19th century, the town became industrialized hosting shipbuilding industries as a result of iron and lumber production. Fishing and whaling were also other economic activities carried out in the town. Then, the area was known as Tom’s River until the mid-1800s. The township was known as a small rural community in the big Dover Township which also had other settlements. Mailing addresses of Toms River were incorporated for Dover Township which led to all mainland being referred to as Toms River. The name was officially changed in 2006 after signatures were collected in a campaign since the name of the township was confusing a lot of people.
The issue of childhood cancer in Toms River Township was a significant problem since the beginning of 1995. The New Jersey Department of Health and the Agency for Toxic Substances and Disease Registry decided to investigate the issue and its correlation to environmental pollution. The two bodies evaluated the rates of the childhood cancer and potential causes such as exposure to hazardous materials. Through a response plan, the community explained their concern over the quality of water they were using which led to the chemical and radiological analysis of the water supply. These analyses discovered that chemicals were being dumped by chemical plants which exposed the community to diseases. The Ciba-Geigy Corporation, Reich farm and the municipal landfill were found to have contributed to the contamination of water supply. The Ciba-Geigy Corporation, which was a chemical manufacturing plant, was closed in 1996 which led to job losses for many residents of the township.

The township has three state representative, the senator and two assemblymen. Senator James W. Holzapfel is a Republican together with Assemblyman John Catalano and Assemblyman Gregory P. McGuckin. The community is represented in the United States Congress by Democrat Andy Kim since 2019.

VULNERABLE POPULATION AND PRACTICE EXPERIENCE.
As a nurse, practice experience is essential. My practice experience is going to take place in the State of New Jersey, Ocean County in Toms River Township at a community restaurant known as JBJ Soul Kitchen. This restaurant located in The B.E.A.T. Center is a product of the JBJ Soul Foundation which was founded by Jon Bon Jovi the artist and his wife Dorothea Hurley. The restaurant serves food to all the needy in society and also the ones who can afford too. Those who are able to purchase meals are required to pay $20 as a donation to the foundation. The needy in the community are all people who cannot be able to fend for themselves especially the homeless and hungry. Since its opening, the restaurants have served more than 100,000 meals with half paying diners and half in-need diners.
Mission.
The JBJ Soul Kitchen looks to provide nutritious locally sourced food in a dining setting to both in-need and able customers as a means of addressing food insecurity in the society. The organization also looks forward to empowering members of the society by offering the available resources and creating volunteer opportunities for those who want to give back. The Kitchen also encourages able customers to pay it forward as a means of covering the expenses of all the in-need customers. It also promises to treat all consumers equally, with respect and dignity, at the same time bringing unity in the community by creating strong and durable relationships through food.

Population Diagnosis and Plan.
Looking at the history of Toms River Township, the area is evidently a cancer cluster. Results have shown that cancer rates are mostly high in young children. The cancer mostly manifested in form of Neuroblastoma which led to physical defects as the children grew up. The cost of managing the cancer is high since it involves chemotherapy and radiology which are quite expensive forms of treatment. Additionally, children and adolescents who have survived Neuroblastoma require very close attention and monitoring due to the side effects of these forms of therapy. Looking at the data above, New Jersey, which hosts Toms River, has a high rate of cancer, including the mortality rate of the same. Additionally, due to the closure of chemical industries and COVID-19, the cost of fending for families and taking care of those affected by cancer has become hard, increasing the number of people in need in this community. This is because the families cannot afford to provide the required care for these patients due to financial constraints and the stay at home directive because of the COVID-19 pandemic. Therefore, the community requires hospice nurses to offer continuous care for the cancer patients and survivors to ensure they are side-effects and pain free.
Target population.
The health plan is going to target Neuroblastoma (cancer) patients and survivors together with their families in the community of Toms River Township. This is because care for cancer patients does not come to a stop even after the end of active therapy.
Collaborative Community Intervention.
To address the gap of continuous care for Neuroblastoma patients in Toms River, there is need to have a well-articulated collaborative care plan. This plan is expected to integrate different disciplines in healthcare so as to cover the gap fully. The intervention will include pediatric oncologists, hospice nurses and home health nurses, primary care nurses and physicians and finally parents, care givers and guardians of the patients. The health education plan will include training on how to handle cancer patients, cancer screening, chemotherapy, radiology and nutrition. This project will go for five years with a goal of seeing more Neuroblastoma survivors and in four years, there will be a 5% reduction in cancer related mortality rate in New Jersey, especially Toms River Township.
SECTION IV
Evaluation.
To determine the effectiveness of the education plan, mortality and recovery data associated with cancer will be collected after each year and compared with previous data on the same. A notable difference on positive outcomes will render the project effective or ineffective. Additionally, the community members will also be expected to put into practice everything taught in the education plan and positive home-based care outcomes will render the project effective.
Facilitators and Barriers.
The project was supported by JBJ Soul foundation who offered meals for all volunteers and interdisplinary teams. The Township Mayor also made the project easier by giving the town hall to be used to conduct trainings. Local hospital oncologists and nurses also participated in the education plan. Poverty was one of the major challenges since many people were unable to attend the training sessions due to lack of transport. Lack of resources such as adequate radiology machines and specialists which made it hard to offer adequate therapy to the patients.
Government representatives and officials will benefit from the information acquired during the planning phase of this community education plan. Additionally, implementing it would assist in reducing cancer incidences and mortality in not only Toms River community, but the state of New Jersey as well. The One notable difference the two groups was the practice experience community I worked with was the homeless and hungry in Toms River since the JBJ Soul Kitchen assisted them through offering their meals and assisting them find resources to change their lives for the better. While, the community diagnosis produced cancer patients who could not access continuous care due to COVID 19 and financial constraints.
Conclusion.
The community of Toms River Township is one that has faced many issues associated with cancer due to pollution over the years. The closure of chemical plants, job loss and COVID 19 has led to poverty, homeless and financial constraints. This has resulted to the need for non profit organizations such JBJ soul foundation to establish their activities in the areas as a means of assisting vulnerable members of the community. However, the issue of mortality and cancer incidences have higher rates still. Therefore, there is need for the community and its legislation to provide adequate continuous care for cancer patients so as to elevate the financial burden and reduce mortality rate of cancer. The cancer is highly affecting young children and adolescents therefore investing in padeatric oncologists and home care nurses or hospice nurses can assist in improving the quality of care. This is a step that Ocean County and New Jersey state can take therefore improving the lives of the society and nation at large.
