• Utilization of materials listed in the Resources above is required.

1. Narrative: Provide a narrative that addresses the identified root causes for the areas of concern and your vision for the school.  (Type your information using double spacing and APA formatting in no more than 2-3 complete paragraphs.)
2. Identify the following within the chart below to create the Action Plan for Year 1-Turn-Around:
   1. A specific, measurable target goal for the subgroup you have chosen.  (SMART Goal)
   2. The strategy for the Turn-Around process. What research says about addressing the identified need and goal in the Turn-Around process.
   3. MDE- resources to begin your project for Turn-around
   4. Who will be responsible for the implementation of strategies?
   5. What is the timeline the focus will cover?
   6. How often will monitoring take place?
   7. What will be the evidence that progress has been made with each of the strategies and goals listed? Remember a strategy is what you plan to do, but the evidence is what you will use to prove the strategy worked.

 Use the template provided in the module, but please refer to the Sample Guide Template provided in the Module. 

Items to submit:

• A copy of your data used in Part A
• The narrative addresses the root cause and the vision for the school.
• Action plans (template) for Year 1: Turn-Around for a particular subgroup in a subject area.
